"Mingalarbar”という言葉は、ミャンマーで日常的に使われる「ご機嫌よう」の意味の挨拶です。
ミャンマーは旧名ビルマとして知られています。有名なクラシック映画である「ビルマの竪琴」は当時の日本人にビルマを知っていただくものでした。
現実と唯一異なっていたのは、僧侶は本来竪琴を弾くことはありません。シナリオや映像も大変興味深く印象に残る内容でした。
ミャンマーは国土が日本の約2倍であるものの、人口は約半分の6千万人です。
しかしながら、歯科大学卒業生は3万人を超えています。軍医大学の歯学部から若干名と、2校ある国立歯科大学のうち、ヤンゴン大学から2,648名、マンダレー大学から973名がB.D.S.の学位を取得しています。
大学では、一般患者へ基本的な歯科医療を行えることを目的に指導しています。ただ残念なことに、大多数が全人口のたった30%が居住する大都市の個人歯科診療に従事しています。
また政府雇用を望み続けている卒業生も一部います。
ミャンマー歯科評議会の記録によると、2,400名が歯科免許を取得し診療に従事しています。
うち403名が国内の様々な医院に勤務し、約1,400名は開業医です。
ミャンマーには、未だ健康保険制度が確立されていません。
抜歯、充填、口腔清掃などの基本診療については、国立病院では無料で行えます。
補綴を含む全ての治療が有償で個人クリニックで行えます。平均的な治療費は抜歯、充填、歯石除去および研磨がUS$10/本、根管治療がUS$50/本、陶歯冠・小さな部分床義歯がUS$100/本、総義歯:US$300などです。
127名の歯科医が大学関係の様々なポジションで活躍しています。2つの大学歯学部長である私とShwe Toe教授の2名は、ともに1年間東京医科歯科大学において、私は1996年に高齢者歯科学、マンダレーの歯科大学のShwe Toe教授は2000年に口腔病理学の教育を受けました。
東京医科歯科大学での先生方との忘れがたい思い出
大学勤務歯科医は勤務時間後と週末には自由に診療することを許可されています。
大学勤務歯科医は低賃金の給与でありながら、各々の理由で満足感を持ち働いています。名誉職である教師、キャリアが積める、そして政府系の被雇用者にのみ許されている研究生として勉強をさせて貰えるチャンスがあるのです。
大学入学試験は今のところ存在しておらず、高校時にそれぞれが選択した全国レベルの試験スコアにより入学が決められます。最も人気のある専門分野は医学で大変競争率が高く、続いてどうにか歯学になります。
専門職に必要な人材に照らして、女性の入学には制限があります。大抵男子生徒より女子生徒の方がスコアが高いので、医科大学では50％まで、歯科大学は20％までの範囲で入学が許可されます。
授業料は政府が優秀な生徒に対して、本人の経済状況に関わらず勉強を進められるよう大学費用を補助しており、毎月US$1で済みます。
私が行う夕方からの診療所
歯科医師は、専門職が通常もう一段階必要なのに対して、医師や技師同様、一度政府系職に就くと公務員としてみなされます。
現在歯学部卒業生に対しての国家資格試験は存在せず、2校の国立大学では交互に実施する共通試験があるが、それを合格したということのみになります。しかしその歯科医師資格は現状継続教育ポイントが付かないまま、3年ごとに更新されます。ミャンマー歯科評議会は地域の要請により外国人歯科医師が慈善診療を提供できるよう一時的な免許を発行します。
外国人歯科医師は現地の協力者からの推薦と、3ヶ月毎にUS$1500の支援金を準備し、診療の許可申請をすることができます。
現在のところ、2名の日本人と1名の韓国人歯科医師が登録されている。
近い将来、こういった歯科医師の環境が良くなったらいいと願っています。経済的余裕のある人は医療や歯科治療を求めて外国へ出ていくこともあります。
またミャンマーに歯科技工所を設立しようとしている外国企業もあります。
最後になりますが、ヤンゴン歯科大学は日本からの援助を色々と受けております。東京医科歯科大学においては30名以上、他校でも数名の歯科医師が指導を受けました。
そして、江藤一洋先生の呼び掛けと援助により、日本帰国歯科留学生同窓会は45名が名を連ねています。
歯科医療機器は2000年に山田巌先生グループの協力により東京医科歯科大学、また2001年に愛知学院大学より譲り受け、さらに2005年には日本政府と日本の支援者より21セットの診療台や機器を草の根支援として譲り受けました。
歯科学生と患者にとって、大変大きな助けになったことは言うまでもありません。
現在、ヤンゴン歯科大学は将来の2国間の学術的協力と連携を進めるために、東京医科歯科大学、愛知学院大学、九州歯科大学、広島大学、新潟大学と協定を結んでおります。

Biruma no Shi Kha: An Excerpt
"Mingalarbar", a word of greeting meaning best wishes to you, is commonly used in Myanmar. Myanmar was formerly known as Burma. Biruma no Tategoto, a famous classic movie introduced our country to most of Japanese people in those days. But only thing contradicted to the real situation is that the Buddhist monks are not supposed to play the harp "tategoto". The theme and the presentation was quite interesting and impressive.
Myanmar is wider than Japan two times and lesser in population about half, sixty millions only. However the number of dental graduates produced is just more than thirty thousands. Two national dental universities, Yangon and Mandalay conferred B.D.S. degrees to 2648 graduates and 973 graduates respectively with a very few number from the dental department of Military Medical university. The aim of the universities is to produce basic dental surgeons for the grassroot level. Unfortunately, majority is sticking for private practice to the big towns where only 30% of total population is residing. There is also a certain group of graduates waiting for the government employment. According to the records of Myanmar Dental Council, 2400 active dentists are registered for dental license. Among them 403 dentists are working at the various hospitals over the country. About 1400 dentists are private practitioners. Myanmar has no health insurance system at yet. The basic dental treatment such as extraction, filling and oral prophylaxis can be free of charge at the government hospitals. All sorts of treatment including prostheses are out of pocket system at the private dental clinics. As an average fee, US $ 10 for one extraction, one filling, scaling and polishing; US $ 50 for one endodontics; US$ 100 for one porcelain crown, short span partial denture; US $ 300 for complete denture; etc.. A total number of 127 dentists are working with the universities at various positions.
Two top ranking officials, the Rectors of dental universities were trained in Tokyo Medical and Dental University for one year, myself in Geriatric Dentistry at 1996 and Prof. Shwe Toe, University of Dental Medicine, Mandalay in Oral Pathology department at 2000.
Dentists working at the universities are allowed to do private practice in the evenings and weekends. All are working at the universities happily due to some reasons regardless of the low salary: a prestigious teaching job, presence of carrier ladder and opportunity to pursue post graduate study allowed only to government service.
There is no university entrance examination so far. It is decided by the marks scored at the national level high school examination depending upon their choice. The most popular profession is medicine which is very competitive, then only come the dental. The female student intake is limited to 50% in medical and 20% in dental universities in order to adjust the need of profession as they always scored higher marks than male. The fee is only US$ 1 per month because the government bears all the expenses of the university allowing the clever students accessible to the profession regardless of their economic background.
The dentists are regarded as gazetted officers once they joined the government service like medical doctors and engineers whereas other professionals need one more step.
At present there is no national licensure examinations for dental graduates as they are products of only two national universities where the examinations are of common type holding as chief convener alternately by the two. But the dental license has to be renewed every three years time without any continuing education credit points so far. Myanmar Dental Council issued temporary licenses to foreign dentists for charity dental practice with the requests from local partners. The foreigner can also apply for license in dental practice by obtaining the recommendation of local partners and by subsidizing US $ 1500 for three months practice. Up to the present only two Japanese dentists and one Korean dentist are registered. Hopefully their business might be better in the near future. Some affordable people went out for foreign countries for seeking medical and dental treatments. Some foreign companies got interest in establishing the dental laboratory in Myanmar.
Last but not the least, University of Dental Medicine, Yangon has obtained support from Japan in various ways. More than thirty dentists were trained in Tokyo Medical and Dental University and a few were from other universities: forming an association called Japan Dental Alumni Myanmar with 45 members: Prof. Kazuhiro Eto as a patron. Some dental equipment were also received from Tokyo Medical and Dental University with the cooperation of Dr. Iwao Yamada group in 2000, some were received from Aichi-Gakuin University in 2001 and another 21 sets of dental chairs and equipments were also received as grassroot grant aid from Japanese Government and people in 2005. The dental students and patients are benefited by the great help very well.
Now, Yangon Dental University has signed in with many Japan Universities like Tokyo Medical and Dental University, Aichi-Gakuin University, Kyushu Dental University, Hiroshima University, and Niigata University for the future bilateral academic cooperation and coordination.
